Reconstruction of Oral Commissure Contracture in Electrical Burn Injury with a Modified Ventral Tongue Flap

Abstract

Sever burn scar contractures in oral commissure present major reconstructive problem. When there are full-thickness destruction of vermilion, mucosa, skin, and orbicularis muscle by electrical burn, the burn scar contracture alters adjacent structures, results microstomia, displaces the commissure, and distorts of facial animation. A 6-year-old-female child patient had a burn scar contracture caused by 220 V electrical burn injury 2 years ago. She had reconstruction using a modified ventral tongue flap with vermilion transpositional flap. Flap was survived completely, donor site was healed by primary closure. It permited effective release of scar contracture and made the oral commissure to normal position almostly. There was no complication such as flap necrosis, dysphagia, disturbance of enunciation and donor site morbidity for 9 months, follow up period. But mild recurrence of contracture was founded. Remaining structures could be returned to their normal location almostly, and the three-dimensional contour of the oral commissure can be better restored by using this flap in patient with severe burn scar contracture of oral commissure. And we could have improved result in functional and esthetic side. (Journal of Korean Burn Society 2007;10:161-164)
